
An investigation has been launched after a man was found dead at Salford’s HMP Forest Bank Prison.

Thirty-three-year-old Dean Clementson died at HMP Forest Bank Prison on April 1. His death follows that of another inmate in November 2024, after which an ombudsman investigation was launched.
A recent inspection conducted by the HM Inspectorate of Prisons found that the private prison failed to provide a suitable environment for rehabilitation due to high levels of violent incidents.
The report also depicted the prison as dangerous for both staff and inmates in terms of drug use, violence and self harm.
HMP Forest Bank said: “We understand this is a difficult time for Mr. Clementson’s family and friends, and our thoughts remain with them.
“We are fully cooperating with all relevant parties during the investigation – however, as this is ongoing, we are not able to comment further.”
Police said there were no suspicious circumstances.
